Registration Information

The New York State Education Department requires school districts to verify proof of age of incoming kindergarten students:

 

  • Birth Certificate (please black-out all Social Security information before copying or showing it to school personnel)

 

  • Baptismal Certificates, or

 

  • Other legal documents (passport).

 

 

New York State Public Health Law also requires the following:

 

  • Physical examination dated after June 1, 2010.

 

  • Children without the required physical exam will be examined by the school physician at school.

 

  • Complete immunization record.

 

  • No child may attend school without the required certification or evidence of immunization.

 

  • Your child must be at least five years old on or before December 1, 2010.       

 

In addition to the above requirements, you must also complete a registration packet.   The packet includes the following forms: 
 
 
1. Registration Form (2 pages)
 
2. Student Residency Questionnaire
 
3. Kindergarten Registration (Student's Name)
 (this form is used when Kindergarten Registration is scheduled in March.)
 
4. Letter to New Students
 
5. Health History (2 pages)
 
6. Developmental Health History (2 pages)
 
7. Parent Health Letter (re: Section 903 NYS Regulations)
 
8. Medication in School
 
9. Physical Exam Form - Must be filled in completely
 
10. Dental Health Certificate
 
11. Getting to Know Your Child (2 pages)
(Again, this form is used for Kindergarten students only)
