  • 2016-17 Student Safety Information Form
Each year, our Lew-Port families are required to submit student contact information in case of an emergency. This form was previously called the "Yellow Card." This year, we are centralizing the information and ask that you please complete the following questions so that your child's most current information is on record in our District files. During this transition, traditional Yellow Emergency Information cards will also be sent home to families, you only need to fill out the online OR the paper copy. Please note that all information you submit is confidential. You can find the online form here. Thank you.
