Pick a Card, Any CardWith multiple carriers and benefits, please refer to information below to determine which card to use when:- Medical Insurance: Univera ID: You receive a card for each enrolled member of your family (each card will arrive in a separate envelope).
- Vision coverage: Davis Vision ID: You will receive a separate set of cards, in your name (the subscriber) only (it is an Univera ID card that says: Vision Plan)
- Pharmacy Insurance: Pharmacy Dimension Benefits (PBD) ID: You receive one set of cards with all family members listed.
- Wellness Benefit: Activate and access your new Univera Wellness Benefit ($300) by logging in to your Univera member account
- Health Reimbursement Account (HRA): P&A Group card: You will receive an HRA Benefits Card (a debit MasterCard) in your name.
- Dental Insurance (LPUT); Delta Dental: You receive a card in your name (the subscriber) only.
- Dental Insurance (all other employees): Pro Benefits Administrators: Dental Pay Plus: You receive a card in your name (the subscriber) only.
Effective January 1, 2025, CVS Pharmacy will no longer be part of Pharmacy Benefit Dimensions (PBD) network. PBD has mailed out the attached letters to all those who are affected by this change.
Transferring Prescriptions to a New Pharmacy
At Pharmacy Benefit Dimensions, we want to help our members have a seamless transition experience when they change pharmacies. Below are a few points on how to change to a new participating
pharmacy.
1. Select a pharmacy in our network
It’s easy to find a pharmacy that participates with Pharmacy Benefit Dimensions. Our comprehensive pharmacy network includes over 54,000 local, regional, national chain and independent pharmacies. We also offer convenient mail order pharmacy options for certain medications.To review our network pharmacies, go to the “Find a Pharmacy” tab at www.pbdrx.com/find-a-pharmacy and select “Pharmacy.” Or, log into your online account, where your location and plan are part of your search profile.
2. Have existing prescriptions? Here’s how to transfer them:
A. Contact your new pharmacy and ask it to transfer your prescriptions from your old pharmacy.
Some pharmacies offer this service on their websites.
OR
B. Some pharmacies will ask you to contact your health care provider who prescribed your medication and ask for your prescriptions to be sent to your new pharmacy. Be prepared to provide their office with the address and phone number of the new pharmacy.
3. Cancel automatic refills
If you have automatic refills, remember to contact your old pharmacy to unenroll for this service.

You also have access to mail order pharmacies, which send your prescriptions to your home at no cost, which can save you both time and money.
Most maintenance medications or drugs with a 90-day supply are available through mail order pharmacies. Please check your formulary for drugs you may obtain through mail order or contact
